Why the Cause of Anaemia Matters as Much as the Diagnosis

Anaemia is not a diagnosis in itself — it's a finding that points to an underlying cause, and the cause completely determines the correct treatment. Iron tablets will not help B12 deficiency anaemia. B12 injections will not address iron deficiency. Neither will help thalassaemia trait. And treating any of these without addressing the underlying reason for the deficiency may allow a more significant condition to remain undetected.

Iron-deficiency anaemia is the most common type in Malaysia — particularly among women of reproductive age, those with heavy periods, and those with limited dietary iron intake. But it's not the only type, and assuming it's the cause without testing is exactly how people spend months on iron supplements that are doing nothing.

Iron-Deficiency Anaemia — Most Common

Low iron stores reduce haemoglobin production. Common in women with heavy periods, those with low dietary iron, pregnancy, and GI blood loss. Treated with iron supplementation — but the cause of the iron loss also needs to be identified.

B12 / Folate Deficiency Anaemia

Produces large, immature red cells (macrocytic anaemia). Common in strict vegetarians, elderly patients, those on certain medications (metformin, PPIs), and those with absorption issues. Requires B12 injections or high-dose oral supplementation — not iron.

Thalassaemia Trait

Highly prevalent in Malaysia — particularly in Malay, Chinese and Indian communities. Causes mild anaemia that does not respond to iron supplementation. Important to identify before pregnancy for genetic counselling. Iron tablets may cause harm if taken unnecessarily in this group.

Anaemia of Chronic Disease

Occurs as a consequence of chronic conditions including diabetes, kidney disease, autoimmune conditions, and chronic infections. Management focuses on the underlying condition — supplementation alone is insufficient.
