Ear pain is one of those symptoms that is hard to ignore — it's sharp, often constant, and frequently accompanied by a sense of fullness or muffled hearing that feels like someone has turned the volume down on your day. In children it's one of the most common reasons for urgent after-hours clinic visits. In adults, particularly swimmers and those who work in hot, humid environments around Kepong's industrial areas, outer ear infections are a recurring problem.
The clinical priority is identifying whether the infection is in the outer ear canal (otitis externa) or behind the eardrum in the middle ear (otitis media) — because these require different treatments. Outer ear infections are treated with antibiotic eardrops and sometimes a wick to deliver medication deeper into the canal. Middle ear infections are treated with oral antibiotics. If the eardrum has perforated (which causes sudden relief of pain followed by discharge), certain eardrops are actually contraindicated — another reason why a proper examination rather than a pharmacy visit is the right first step.
Our GP examines both ears directly with an otoscope — a bright light and magnifying lens that clearly visualises the ear canal and the eardrum. Treatment is prescribed immediately at the same visit.
The outer ear canal becomes infected — typically by bacteria after water exposure (swimmer's ear), cotton bud trauma, or in humid conditions. Pain is typically worst when the outer ear is touched or when chewing. The canal may be swollen and tender.
Treatment: Antibiotic and/or steroid eardrops, ear wick if the canal is very swollen, and keeping the ear dry. Oral antibiotics where appropriate.
Infection behind the eardrum — most common in children following an upper respiratory tract infection, but adults are also affected. Deep ear pain, reduced hearing, sometimes fever, and occasionally the eardrum ruptures (which actually relieves the pain but produces discharge).
Treatment: Oral antibiotics where appropriate, pain relief, and follow-up to confirm resolution and check hearing has returned to normal.
