Sarah remembers the exact moment she knew something was wrong. It was Tuesday morning, four days after a date that ended at her flat in Mont Kiara. She was getting ready for work, went to the bathroom, and felt it — that sharp, stinging burn that made her flinch.
“Maybe it’s a UTI,” she thought. She drank more water, tried cranberry juice from the 7-Eleven downstairs, and powered through her workday. By Thursday, the burning was worse and she’d started feeling a dull ache in her lower belly. That’s when she finally booked an appointment.

Why Painful Urination After Sex Is Different

Painful urination has a medical name — dysuria — and it has many possible causes. A normal UTI from holding your pee too long during a meeting at KLCC, a urinary stone, even harsh soap. But when the burning starts within days or weeks after unprotected sex, the most likely cause is a sexually transmitted infection (STI) affecting your urethra.
This is true for both men and women, though the symptoms can feel slightly different. Men often describe it as a sharp sting at the tip of the penis when peeing. Women tend to feel it lower, almost like an internal burn, sometimes with extra pressure as the bladder empties.
Either way, the message is the same — your urinary tract is inflamed, and there’s likely a reason for it that won’t resolve on its own.
STIs That Commonly Cause Burning Urination

Three infections account for the vast majority of post-sex dysuria cases we see at our clinic in Klang Valley:
Chlamydia.
Often called the silent STI, chlamydia is by far the most common cause. The burning is usually mild to moderate, and it shows up about 1 to 3 weeks after exposure. About half the time there’s no other symptom — just the burning. This is what Sarah turned out to have.
Gonorrhea.
More aggressive than chlamydia. The burning sensation is usually intense — patients describe it like “peeing glass.” It typically appears 2 to 7 days after exposure and often comes with thick yellow or green discharge from the penis or vagina.
Trichomoniasis.
This parasite causes itching alongside burning urination, often with frothy or unusual discharge. It’s more common in women but men can carry it without symptoms and pass it on.
Other possibilities include genital herpes (which causes painful peeing during outbreaks because urine touches the sores), and in rare cases, syphilis. The point is — there are several different causes, and only a proper test can tell them apart. Treatment for one isn’t always treatment for another.
How Soon After Sex Can Symptoms Appear?
This is one of the most common questions we get, especially from anxious patients who suspect something but aren’t sure if it’s too early to test. Here’s a rough guide for the most common infections:
- Gonorrhea — symptoms typically 2 to 7 days after exposure
- Chlamydia — symptoms typically 1 to 3 weeks after exposure (sometimes longer)
- Trichomoniasis — symptoms typically 5 to 28 days after exposure
- Herpes — first outbreak usually 2 to 12 days after exposure
- UTI from sexual activity — usually within 24 to 48 hours
Some people get symptoms much earlier or later, and many people get no symptoms at all. If you’ve had unprotected sex and have any reason to be concerned, the testing window starts much earlier than the symptom window. Most STI tests at our clinic can detect infection within 1 to 2 weeks of exposure, even before symptoms appear.
It Might Just Be a UTI—But Don't Assume
Yes, urinary tract infections happen after sex too, particularly in women. The mechanical action of intercourse can push bacteria from the genital area into the urethra. This is sometimes called “honeymoon cystitis” and it’s very common.
The problem is — UTI symptoms and STI symptoms overlap almost completely. Burning urination, frequent peeing, lower abdominal discomfort. Without a proper test, there’s no way to tell them apart by symptoms alone. We’ve seen patients who self-treated with cranberry supplements for weeks while a chlamydia infection quietly travelled up their reproductive system.
If your symptoms started within days of unprotected sex, please don’t gamble. A simple urine test can sort it out in 1 to 3 days, and the right treatment depends entirely on the right diagnosis.
What Happens If You Ignore It?
Untreated chlamydia or gonorrhea is one of the leading preventable causes of fertility problems in Malaysia. In women, the infection can travel from the urethra and cervix up into the fallopian tubes, causing pelvic inflammatory disease (PID). PID can cause scarring that leads to chronic pelvic pain, ectopic pregnancy, or infertility — sometimes years later, long after the original infection has been forgotten.
In men, untreated infection can spread to the testicles (epididymitis) or prostate, causing pain, swelling, and in rare cases, fertility issues. Some untreated infections also significantly increase the risk of acquiring HIV from a future exposure.
None of this is meant to scare you. The good news is that most of these infections are completely curable with the right antibiotics, and the earlier they’re caught, the easier the treatment.
