Aiman noticed it three days ago — a small wet patch on his underwear when he got home from work. At first he thought maybe he’d spilled something. But then it happened again the next morning. By day three, there was a yellowish-white stain on his briefs and a faint burning when he urinated.
“Mungkin nothing lah,” he told himself, scrolling through his phone in the LRT on the way to Mid Valley. But that night, lying in bed, he finally typed it into Google: “white discharge from penis Malaysia.” The search results made his stomach drop.

Is Any Discharge from the Penis Normal?
Short answer: not really. Unlike women, men don’t typically have any regular discharge from the urethra outside of urine, semen during ejaculation, and a small amount of clear pre-ejaculate fluid during arousal. That’s it.
So if you’re noticing fluid that isn’t connected to sexual activity — especially if it’s white, yellow, green, or cloudy — that’s your body sending a signal. Something needs attention. The most common culprits in Malaysian men are sexually transmitted infections (STIs), but other conditions like urinary tract infections (UTIs) or prostatitis can also cause discharge.
The colour, smell, and texture can give us early clues, though only a proper test can confirm what’s actually going on.
Common Causes of White Discharge in Men
Let’s go through the most likely culprits, especially among men in KL and Selangor based on what we see at our clinic.
Chlamydia.
This is the number one cause of male urethral discharge in Malaysia, particularly among men aged 18 to 35. The discharge is usually clear or milky white, and it often comes with a mild burning sensation when peeing. The tricky thing about chlamydia? Up to half of men have very mild symptoms — or no symptoms at all. Many men only find out they have it after their partner gets tested.
Gonorrhea.
Gonorrhea typically produces thicker, yellow-green discharge and a much more obvious burning sensation when urinating. The symptoms usually appear 2 to 7 days after exposure. Unlike chlamydia, gonorrhea tends to make itself known — but it has also become increasingly resistant to common antibiotics, which is why proper testing matters more than ever.
Non-gonococcal urethritis (NGU).
Sometimes the discharge is caused by other bacteria like Mycoplasma genitalium or Ureaplasma. These don’t show up on standard chlamydia and gonorrhea tests, so if your initial results come back negative but symptoms persist, your doctor may run additional tests.
Trichomoniasis.
Less common in men but worth mentioning — this parasite-caused infection sometimes produces discharge along with itching inside the penis. Many men carry it without symptoms and unknowingly pass it to partners.
Urinary tract infection (UTI).
UTIs are less common in men than women, but they do happen — especially in older men or those with prostate issues. Symptoms can mimic an STI.
Symptoms That Mean You Should See a Doctor This Week
Don’t wait it out if you’re experiencing any of these:
- Any unusual discharge from the penis (clear, white, yellow, or green)
- Burning, stinging, or pain when urinating
- Increased frequency of urination
- Itching or irritation inside the penis
- Pain or swelling in the testicles
- Discomfort during ejaculation
- Sores, bumps, or rashes on or around the genitals
And if your partner has been diagnosed with an STI — even if you have zero symptoms — you should still get tested. STIs in men are notoriously sneaky.
Why Men in KL Delay Getting Checked (And Why That's Risky)
We see this pattern all the time. The symptoms appear, the patient panics for a day or two, and then… life happens. Work gets busy. The discharge seems to lessen. Maybe it’ll just go away on its own?
Here’s the thing — even when symptoms fade, the infection often doesn’t. Untreated chlamydia and gonorrhea can travel up the urinary tract and cause epididymitis (a painful inflammation of the testicles), prostatitis, and in rare cases, fertility problems down the line. Untreated infections can also be passed to current or future partners without you knowing.
