TSH (thyroid stimulating hormone) is the primary screen. If abnormal, Free T4 and Free T3 are added to characterise whether the thyroid is over or underactive. In women, both low and elevated testosterone cause problems — low levels affect energy and libido while elevated testosterone (common in PCOS) is associated with irregular periods, acne, and unwanted hair growth. A blood test measuring total testosterone, and where indicated free testosterone and SHBG, gives your doctor the information needed to assess whether your levels are clinically significant and advise on management. The ANA (antinuclear antibody) test screens for autoimmune conditions — diseases where the immune system mistakenly attacks the body's own cells and tissues. It's ordered when a patient has symptoms that suggest an autoimmune cause: persistent joint pain and swelling that doesn't have an obvious mechanical explanation, unexplained skin rashes (particularly a facial butterfly rash), recurring mouth ulcers, fatigue significantly out of proportion to activity, and hair loss without apparent cause.
A positive ANA doesn't automatically confirm a specific autoimmune disease — it indicates that further investigation is warranted. Your doctor interprets the result in the context of your full clinical picture. Conditions it helps screen for include lupus (SLE), rheumatoid arthritis, Sjögren's syndrome, scleroderma, and mixed connective tissue disease. If you've had these symptoms for months and don't have a diagnosis, this test is a reasonable first step.
