Chickenpox (varicella) is caused by the varicella-zoster virus and spreads extremely easily — through direct contact, respiratory droplets, and even through the air. It is so contagious that before vaccination was available, nearly every child was infected before adulthood. The characteristic itchy, blister-like rash typically affects the entire body and is accompanied by fever and malaise lasting 5–10 days.
While often dismissed as a routine childhood illness, chickenpox carries real risks — particularly for certain groups. Bacterial skin infections from scratching are common, even in otherwise healthy children. More serious complications include pneumonia, encephalitis (brain inflammation), and cerebellar ataxia (coordination problems). Chickenpox in adults is significantly more severe than in children. Pregnant women who contract chickenpox risk congenital varicella syndrome in the baby, or severe neonatal chickenpox if infected near delivery.
Beyond the acute illness, the varicella-zoster virus stays dormant in nerve tissue for life after infection — and can reactivate decades later as shingles, a painful blistering rash along a nerve distribution. Vaccinated individuals have a significantly reduced risk of developing shingles later in life. The Zoster vaccine for adults is a separate consideration for those over 50.
Single dose for children 12–15 months. Often given at the same visit as MMR. Provides approximately 90% protection against any chickenpox and over 98% protection against severe disease.
Adults who have never had chickenpox or been vaccinated should receive 2 doses — given 4–8 weeks apart. Particularly important for women planning pregnancy, healthcare workers, and those in close contact with high-risk individuals.
A single unvaccinated child with chickenpox in a classroom will typically infect 80–90% of unprotected classmates. This high transmissibility is why chickenpox outbreaks in nurseries and schools still occur commonly among unvaccinated groups.
Chickenpox in the first 20 weeks of pregnancy can cause congenital varicella syndrome — affecting the baby's skin, limbs, eyes, and brain. Chickenpox just before delivery can cause severe neonatal infection requiring urgent treatment. Women who are not immune should be vaccinated before becoming pregnant.
Vaccinated individuals carry a significantly lower varicella viral load in their nerve tissue compared to those who had natural infection — meaning their lifetime risk of shingles is substantially reduced. Natural infection does not provide this advantage.
Chickenpox in adolescents and adults causes more severe symptoms, higher rates of pneumonia, more extensive rash, and longer recovery than in young children. Adults who never had chickenpox and are not immune should be vaccinated.
The chickenpox vaccine is commonly given alongside the MMR vaccine at 12 months — two separate injections at the same visit. This is safe, standard practice and ensures both vaccines are on schedule without requiring an extra clinic visit.
