What Is Pertussis — and Why Is It Especially Dangerous for Infants?

Pertussis — commonly called whooping cough — is a highly contagious bacterial respiratory infection caused by Bordetella pertussis. It spreads through respiratory droplets and is one of the most transmissible infectious diseases known: a single unprotected individual in a household with pertussis will typically infect 12–17 others without vaccination providing a barrier.

The disease gets its name from the characteristic high-pitched "whoop" sound that follows violent coughing fits as the patient gasps for air. In older children and adults, pertussis is miserable but rarely fatal — the cough can last 6–10 weeks and is intensely disruptive to sleep and daily life. In infants under 6 months, however, the disease is a medical emergency. Young infants often cannot generate the classic whooping sound — instead they experience prolonged apnoeic episodes (pauses in breathing), cyanosis (turning blue), and can die from respiratory failure.

Vaccination through the childhood DTaP series is the most effective protection. Pregnant women are also recommended to receive a Tdap booster in the third trimester — the antibodies pass to the newborn before they are old enough to be vaccinated themselves.

Infants Can Stop Breathing During Coughing Fits

Young babies lack the respiratory muscle strength to manage violent pertussis coughing. Episodes can cause oxygen deprivation, brain damage, and death — often in previously healthy infants with no warning signs.

🌬️

Spreads Before Symptoms Are Obvious

Pertussis is most contagious in the early "cold-like" stage — before the characteristic cough develops. This means infected family members unknowingly expose newborns before anyone realises what the illness is.

🤰

Pregnant Women Should Get a Tdap Booster

A Tdap vaccine in the third trimester passes maternal antibodies to the baby in utero — providing passive protection from birth until the infant is old enough to start their own vaccination series at 2 months.

👨‍👩‍👦

Household Contacts Should Be Vaccinated

Anyone who lives with or regularly cares for a newborn — parents, grandparents, siblings — should have up-to-date pertussis coverage. Infection in adults is often mild but easily transmitted to the unprotected infant.

💡

Immunity Wanes in Adolescence and Adulthood

Childhood DTaP protection fades over time. Adolescents and adults can and do contract pertussis — and then spread it to infants too young to be vaccinated. A Tdap booster restores protection.

Pertussis Vaccination — Frequently Asked Questions

Common questions from parents in Damansara Jaya and the wider Petaling Jaya area about whooping cough vaccination.

If your baby is under 6 months and has had a persistent cough for more than a week — especially with episodes of prolonged coughing, a high-pitched gasp after coughing, or any pause in breathing — come in immediately. Our clinic is open 24 hours. Pertussis in young infants can escalate to a medical emergency rapidly. Even if it turns out to be something else, the assessment is worth it.
Yes — this is strongly recommended. A Tdap booster given in the third trimester (27–36 weeks) allows maternal antibodies to pass to the baby through the placenta, providing passive protection from birth until the infant starts their own vaccination series at 2 months. This is the most effective way to protect newborns during the most vulnerable window. Speak to our doctor about Tdap in pregnancy.
Yes. Immunity from childhood DTaP vaccination wanes significantly by adolescence and further into adulthood. Adults can and do contract pertussis, typically experiencing it as a prolonged, persistent "100-day cough" — often misdiagnosed as bronchitis or post-viral cough. Adults with pertussis readily spread it to unprotected infants. A Tdap booster restores protection — ask our doctor about your booster status.
No. In Malaysia's national schedule, pertussis is not given as a standalone injection. It is combined with diphtheria, tetanus, polio, and Hib in the DTaP-IPV-Hib 5-in-1 vaccine — a single injection at 2, 3, and 5 months, with booster doses at 18 months and 6 years. No separate visit or additional injection is required specifically for pertussis.
