What Is Hib — and Why Is It One of the Most Important Childhood Vaccines?

Haemophilus influenzae type b — commonly called Hib — is a bacterium that, despite its name, has nothing to do with influenza. Before the Hib vaccine became widely available in the early 1990s, it was the leading cause of bacterial meningitis in children worldwide, and a significant cause of severe pneumonia, epiglottitis (swelling of the throat flap that can cause airway obstruction), and septicaemia in children under 5 years of age.

Hib spreads through respiratory droplets from carriers — many of whom show no symptoms themselves. Young children, particularly those under 2 years, are most susceptible because their immune systems have not yet developed the ability to respond effectively to the bacterial capsule. This is precisely why the vaccine is given so early in the schedule — starting at 2 months — before the window of highest risk opens fully.

Since the introduction of widespread Hib vaccination, cases of Hib meningitis have fallen by over 95% in countries with high vaccine coverage. In Malaysia, it is included in the national immunisation programme as part of the 5-in-1 DTaP-IPV-Hib vaccine — meaning no additional injection is needed beyond the standard childhood schedule.

Third and final primary dose — DTaP-IPV-Hib 5-in-1

🧠

Leading Bacterial Cause of Meningitis in Children Under 5

Hib meningitis — inflammation of the membranes surrounding the brain — carries a 5% mortality rate even with treatment, and 20–30% of survivors develop permanent neurological complications including deafness and brain damage.

😮‍💨

Can Cause Epiglottitis — a Life-Threatening Airway Emergency

Hib infection of the epiglottis causes rapid, severe swelling that can completely obstruct the child's airway within hours. Before vaccination, Hib epiglottitis was a genuine paediatric emergency presenting predominantly in 2–5 year olds.

📉

95%+ Reduction in Hib Disease Since Vaccine Introduction

Countries with sustained high Hib vaccination coverage have seen invasive Hib disease fall by over 95%. This is one of the most dramatic success stories in vaccine-preventable disease control in paediatric medicine.

💉

No Booster Needed — 3-Dose Primary Series Provides Lasting Protection

Unlike some vaccines, the 3-dose Hib primary series at 2, 3, and 5 months provides durable long-term protection without a scheduled booster dose. This makes compliance with the early childhood schedule particularly impactful.

💡

Included in the 5-in-1 Injection — No Extra Shot

Hib is combined with diphtheria, tetanus, pertussis, and polio in a single injection. Your child receives protection against five serious diseases from one injection at each visit at 2, 3, and 5 months.

What to Expect at Your Hib Vaccination Visit

Hib is included in the standard childhood 5-in-1 injection — no separate appointment or additional shot required.

Step 01🚶

Walk In

No appointment needed. Let our team know your child's age and which dose is due. Bring the vaccination card if available.

Step 02👨‍⚕️

Doctor Review

Your doctor confirms your child is well, checks vaccination records, and confirms the appropriate dose and schedule for the 5-in-1 series.

Step 03💉

5-in-1 Administered

Single intramuscular injection into the outer thigh (infants) or upper arm covering Hib alongside diphtheria, tetanus, pertussis, and polio simultaneously.

Step 04📋

Record Updated

Dose recorded in your child's digital health record and vaccination card. Next dose timing confirmed before you leave. Third dose at 5 months completes the Hib series.

Hib Vaccination — Frequently Asked Questions

Common questions from parents in Damansara Jaya and surrounding Petaling Jaya areas about Haemophilus influenzae type b vaccination.

No — despite the name, Haemophilus influenzae has nothing to do with influenza (the flu virus). It was named in the early 20th century when it was incorrectly believed to cause influenza. It is in fact a bacterium, not a virus, and causes a completely different range of serious infections — primarily meningitis, pneumonia, and epiglottitis — particularly in young children. The influenza vaccine is a separate vaccination that protects against the actual flu virus.
In Malaysia's current national schedule, no booster dose is required after the 3-dose primary series at 2, 3, and 5 months. The primary series provides durable long-term protection. Some countries use a 4-dose schedule with a booster at 12–15 months — your doctor will advise if this is recommended in your child's specific situation.
In infants and young children, signs of bacterial meningitis include: high fever, unusual drowsiness or difficulty waking, persistent inconsolable crying, bulging fontanelle (the soft spot on the top of the head), stiff neck, sensitivity to light, and a non-blanching rash (a rash that does not fade when pressed with a glass). If your child shows any of these — especially the non-blanching rash — this is a medical emergency. Bring them to our clinic immediately or call emergency services.
Yes. The catch-up schedule depends on your child's current age. Children under 12 months who have missed doses can still complete the primary series. Children over 12 months who have had no Hib doses typically only need 1 dose as the immune system matures and a shorter schedule suffices. Children over 5 years generally do not require Hib vaccination. Walk in with your child's vaccination card and our doctor will determine exactly what is needed.
