What Is PCV — and Why Pneumococcal Protection Matters for Young Children?

The Pneumococcal Conjugate Vaccine — PCV — protects against Streptococcus pneumoniae, a bacterium that is one of the leading infectious causes of serious illness and death in children under 5 years worldwide. Pneumococcal bacteria can cause three particularly dangerous conditions: pneumonia (lung infection), bacterial meningitis (infection of the brain lining), and bacteraemia or septicaemia (bacteria in the bloodstream) — any of which can be rapidly life-threatening or leave permanent complications including deafness and brain damage.

What makes pneumococcal disease particularly challenging is that many people carry the bacterium harmlessly in their nose and throat — so children are frequently exposed through normal contact in households, nurseries, and community settings. Young infants, whose immune systems cannot yet effectively mount a response to the bacterial capsule, are most vulnerable. This is why PCV is started at 2 months of age — when protection is most urgently needed.

PCV13, the formulation used in Malaysia's national schedule, covers 13 of the most clinically significant pneumococcal strains, accounting for the majority of serious pneumococcal disease. A separate PPSV23 vaccine offering broader strain coverage is available for adults and high-risk groups at our clinic.

However, since pneumococcal pneumonia accounts for a large proportion of the most serious cases requiring hospitalisation, PCV provides very meaningful protection even though it isn't a universal pneumonia vaccine.
Yes. The catch-up schedule depends on the child's current age and how many doses have been received. Children under 12 months who have missed doses can complete the primary series. Children aged 12–23 months who have had fewer than 3 doses will receive a different number of catch-up doses. Children 24 months and older who are at high risk may need 1–2 doses.
